Bone Cancer in Big Dogs: Don't Wait Out the Limp

Osteosarcoma is the most common bone cancer in dogs, and in large breeds a sudden limp can be the first sign. What to watch for, how it's diagnosed, and the treatments, including a newly approved immunotherapy.

Few words hit a dog owner harder than a bone-cancer diagnosis. But looking away from the possibility doesn’t lower the odds, and with this disease, catching the early signs is exactly what widens your options. Recognizing what you’re seeing, and acting on it, genuinely changes how these cases go.

Osteosarcoma is a bone tumor and one of the five most frequently diagnosed cancers in dogs. It also has a fairly predictable profile: it clusters in certain sizes, ages, and breeds, which means you can gauge your own dog’s risk instead of worrying in the dark.

What osteosarcoma is

Osteosarcoma is the most common bone cancer in dogs, accounting for about 85 percent of all skeletal tumors, or roughly 25,000 cases a year in the United States. It tends to be aggressive. Most often it shows up in a front limb, just above the “wrist,” or in the tibia, fibula, or femur of a hind leg, though it can also appear in other bones and is sometimes found in the vertebrae or pelvis.

Age, size, and sex

Risk climbs with age and size. Data presented at a 2016 veterinary conference described prevalence rising into late middle age and peaking roughly between eight and 11 years, then tapering off as big and giant-breed dogs approach the end of their normal life expectancy. In plain terms: a big dog is most vulnerable to osteosarcoma during those middle years. Sex plays only a minor role, with males slightly more affected than females.

Which breeds are predisposed

Selective breeding for particular traits can also carry along a higher genetic risk for certain diseases, and osteosarcoma is a well-studied example. It lands hardest on large and giant breeds, including the Irish Wolfhound, Greyhound, Akbash, St. Bernard, Leonberger, and Rottweiler.

“The single most useful thing an owner of a big dog can do is treat a new, unexplained limp as a reason to call, not a reason to wait a week,” says Dr. Mara Chen. “In a giant breed over seven, I would much rather see a dog and rule bone cancer out than have someone assume it’s a sprain.”

Signs and symptoms

Watch how your dog moves. If there’s no clear reason for a sudden struggle to walk or a favored leg, take it seriously. Swelling and pain are common, and as the pain builds it can sour a dog’s mood, so a normally easygoing pet who turns irritable and reluctant to move deserves a look. The signs to keep in mind:

  • Trouble walking
  • Favoring one leg
  • Swelling
  • Pain
  • Irritability

Some families only discover the tumor because the weakened bone fractures. You don’t want it to reach that point. By the time it announces itself through a break, it may already have spread to the lungs, which makes treatment much harder.

How it’s diagnosed

X-rays are the first test. On film, osteosarcoma tends to give the bone a moth-eaten look, described in vet shorthand as lytic, because of the loss of normal bone. A few far less common conditions can mimic that appearance, so a definitive answer usually requires a fine-needle aspirate or a biopsy, a tissue sample drawn directly from the tumor with the dog under sedation.

Once a diagnosis is confirmed, staging comes next, to learn how far things have progressed. That typically means bloodwork, urinalysis, chest X-rays to check the lungs, and an abdominal ultrasound. If lymph nodes are enlarged, they may be biopsied too.

How it’s treated

Because osteosarcoma moves fast and spreads readily, treatment can be aggressive.

Amputation and chemotherapy

The standard recommendation is to amputate the affected limb and follow with a course of chemotherapy. Amputation isn’t always feasible, though, depending on where the tumor sits and the dog’s overall condition.

Radiotherapy for pain

When surgery isn’t the right call, the focus shifts to comfort. In a long-referenced overview of the disease, veterinarian Wendy Brooks explained that the best surgical candidates are dogs with a single leg tumor and no visible spread to the lungs, while a dog whose remaining legs carry heavy arthritis, or whose cancer has already reached the chest, may do better keeping the leg and leaning on radiotherapy for pain control instead of amputation.

ELIAS cancer immunotherapy

For some dogs, an option beyond the classic surgery-and-chemo path now exists. ELIAS cancer immunotherapy (ECI) grew out of a first-of-its-kind study launched in 2017 by researchers at the University of Missouri College of Veterinary Medicine, who set out to build a patient-specific treatment for canine bone cancer. Their approach created a personalized vaccine from the dog’s own osteosarcoma that, unlike chemotherapy, targeted cancer-specific cells and left healthy ones alone.

The protocol also ran fast: about eight weeks, versus the many months conventional chemo can take. According to the results published in the Journal of Veterinary Internal Medicine, among 14 dogs treated with ECI, median survival was 415 days, and five dogs lived longer than 730 days. All had undergone limb amputation first, but none received chemotherapy.

Work like this is part of comparative medicine, in which scientists study the overlap between canine and human cancers to improve prevention, diagnosis, and treatment for both. For osteosarcoma, comparing the genetic profiles across the two species turned up something striking: they looked essentially indistinguishable. In this disease, at least, we really are a lot like our dogs.

What causes it

Researchers at the University of Wisconsin–Madison School of Veterinary Medicine may have pinpointed part of the biological machinery behind tumor formation. Comparing tumor-forming and non-tumor-forming cell lines, they found several hundred genes behaving differently between the two. One protein, frizzled-6, stood out: it appeared at levels roughly eight times higher in the cells that went on to form tumors. Proteins like this help run essential cellular jobs, such as receiving signals and switching on growth pathways, and when those pathways misfire they may help tumors take hold. The link is still preliminary, and frizzled-6’s exact role isn’t settled, but if researchers can confirm it drives tumor-forming cells, it could give oncologists a new therapeutic target for both dogs and people.

What’s new since this was written

The biggest development is that the immunotherapy above stopped being purely experimental. In March 2025, ELIAS Animal Health announced that its ELIAS cancer immunotherapy received full approval from the USDA’s Center for Veterinary Biologics, making it the first autologous (made from the individual patient’s own cells) prescription product to earn that clearance. It’s now offered through a network of authorized treatment centers around the country, so it is a real conversation to have with a veterinary oncologist rather than a trial you have to chase.

Researchers are also pushing to spare the leg. A study backed by the Morris Animal Foundation and run with the University of Missouri is testing ECI combined with a new adjuvant as a limb-sparing option for large-breed dogs with appendicular osteosarcoma, and additional chemo-immunotherapy data continued to be published through 2025. None of this makes bone cancer easy, but the menu of options is wider and more personalized than it was even a few years ago. If your dog is diagnosed, ask specifically about immunotherapy and limb-sparing approaches alongside the standard path.

This is a serious diagnosis, and every dog’s case is different. Use this as background for the conversation with your own veterinarian or a veterinary oncologist, not as a substitute for it.
